Entry Information - The following must be included with the entry form:
- 1) Poplar Place Release Form,
- 2) GDCTA's Hold Harmless Statement,
- 3) a copy of the horse's Coggins, and
- 4) payment
No fax or email entries will be accepted without prior permission from show management. Fax and email entries will be held responsible for all fees.
Fillable Entry Form - Requires Adobe Reader software (Vrs 8 or later).
Fill in fields then PRINT and mail/fax. Or you can scan the printed form back to your computer and then send as an email attachment. However, you cannot save or email this form if you only have Adobe Reader. You must have either Adobe Standard or Professional to save/email a copy of the form.

Scheduling Note: In general, dressage Combined Tests/3-Phase will be scheduled first, starting with Amoeba and working up the levels. Show jumping and Combined Tests jumping rounds will be scheduled together (i.e. Show Jumping 18" and Amoeba CT Jumping). Dressage only tests are scheduled after the Combined Tests. All dressage tests (CT and dressage only) requiring a standard (large) arena are scheduled after all other dressage tests. Adjustments are made, as required, to minimize scheduling conflicts.
